Our Complaints Management Policy

The Artlife Studio at Footscray Community Arts is a registered NDIS service provider. Any person has the right to submit feedback or make a complaint about a registered NDIS service provider.

You can give feedback about the Artlife Studio program, or make a complaint, at any time. FCA will make sure that we acknowledge your feedback or complaint, look into it, and take any appropriate actions to fix the problem. We will make sure your feedback or complaint is kept confidential, and we will share with you the steps we take to look into your feedback or complaint, and the reasoning behind any actions or decisions we make.

You can give feedback or make a complaint anonymously, using the form below. If you choose to be anonymous, we won’t be able to contact you to let you know what steps we took in response to your feedback or complaint.

If you call or email us, we won’t be able to keep your feedback or complaint anonymous, but we will still look into it and let you know what steps and decisions we make.


The NDIS Commission

You can also submit your complaint to the NDIS Quality and Safeguards Commission. The NDIS Commission works with participants and providers to improve the quality and safety of NDIS services and supports. You can make a complaint to the NDIS Commission here.
